Doctor House Calls in Staten Island, Brooklyn, and Queens.
House calls gives the doctor a unique opportunity with patients that they don’t get in an office visit or during hospitalization. A house call fosters the doctor/patient relationship and improves the doctor’s understanding of the patient’s environment and support systems.
WE PROVIDE primary care, well visits, sick visits, chronic disease management, and palliative care to support your ongoing continuity of care.

Who is eligible?
-
Patient is homebound.
-
Patient is not mobile or has difficulty ambulating.

Patient is using some mobility devices.

Patient is disabled or has difficulty visiting a Doctor’s Office.

Physician needs to assess home environment, and patient and caregiver function.
We help to fill out paperwork required for obtaining or maintaining home health aid assessment for Physical Therapy and Occupation Therapy services assessment for need for end of life care/palliative care and hospice care.
Will My Insurance Cover the House Calls?
Most medicare plans cover house calls for people who are homebound.

If your insurance is not covering house calls, we provide cash services (initial visit $175 and follow-up visit $150)
